CHRONIC CARE INITIATIVE
It’s estimated that 60 percent of San Diegans are living with at least one chronic health condition. Since 2014, local healthcare advocates have worked collaboratively with policy makers, industry leaders, providers, and consumers to ensure access to care and treatment for San Diegans with chronic health conditions. Starting January 1, 2022, our local organizing efforts under the umbrella of the California Chronic Care Coalition changed to Patient Advocates United.
