Nearly twenty-five years ago San Diego became a staffed chapter of the American Liver Foundation. During that time, local volunteers made a measurable difference in the fight against liver disease by providing financial support for medical research; educating patients, families, caregivers and healthcare professionals; advocating for patients and their families; and creating public awareness campaigns about liver wellness and disease prevention.

On July 1, 2020, as a result of COVID-19, the American Liver Foundation closed all their field offices and had a staff reduction of 43 percent. Although ALF would remain a national voice on liver disease, this left a gap in community-based services for San Diego. Local medical specialists, transplant surgeons, patients, and caregivers realizing this gap organized to make sure that these services would continue and be provided locally.  The Liver Coalition of San Diego was incorporated as a nonprofit on July 21, 2020, with the California Chronic Care Coalition serving as its fiscal sponsor until being designated as a tax exempt 501(c)(3) organization by the IRS on October 21, 2020.

On May 24, 2022 the Liver Coalition® became a registered trademark. August 1, 2022 the Liver Coalition® started expanding nationally through its Community Chairs Initiative. Building off the success in San Diego County. Funds raised through this initiative support community-based educational programs and services that benefit local communities.
